## About

InnoCre Telehealth is a HIPAA-compliant virtual care practice founded by Atul S. Vellappally, DNP, CRNP, FNP-BC (NPI: 1497401996). We are a relationship-aware tele-urgent and bridge care provider — keeping patients covered between doctors and between visits. Every patient receives an individualized care plan informed by their genetics, lifestyle, labs, and medical history — not a protocol-based checklist.

## Who We See

InnoCre treats adults and adolescents age 12 and older who are physically located in Delaware, Maryland, or Washington at the time of their visit. We do not see children under 12 or patients located outside our licensed states.

- Urgent care for non-emergency issues (UTIs, sinus infections, rashes, colds, flu)
- Bridge prescriptions and medication refills when patients are between providers
- Chronic care check-ins (diabetes, hypertension, thyroid monitoring)
- STD testing coordination and discreet treatment
- Lab orders, result reviews, and specialist referrals with follow-through
- Women's and men's health consultations
- Care navigation and coordination with in-person providers

## What We Do Not Do

- We do not prescribe controlled substances (opioids, benzodiazepines, stimulants, antipsychotics)
- We are not a replacement for emergency care — call 911 for emergencies
- We will always refer to in-person care when it is more appropriate

## Trust & Safety

- HIPAA compliant; encrypted in transit (TLS) and at rest (AES-256)
- Patient health data is never sold, shared with advertisers, or used for marketing
- No ad tracking on clinical pages
- Conservative prescribing policies — no Schedule II–V controlled substances via telehealth; also no antipsychotics or mood stabilizers
- EHR vendor (CharmHealth) is HITRUST CSF certified and undergoes regular independent audits
