1. Your Rights
As a patient of Innocre Telehealth, you have the right to:
- Receive respectful, dignified care regardless of race, ethnicity, gender, sexual orientation, age, disability, religion, or socioeconomic status
- Be fully informed about your diagnosis, treatment options, and prognosis in language you can understand
- Participate in decisions about your care, including the right to refuse treatment
- Access your medical records and request copies in accordance with applicable law
- Have your protected health information kept confidential under HIPAA and applicable state laws
- Receive care from licensed, credentialed healthcare providers
- Know the name, credentials, and qualifications of your treating provider
- Receive a clear explanation of all charges and fees before services are rendered
- File a complaint or grievance without fear of retaliation or impact on your care (see our Complaints & Grievance Process)
- Withdraw consent for telehealth services at any time
- Request a referral for in-person care at any time
- Receive timely responses to questions and concerns about your care
2. Your Responsibilities
As a patient, you are responsible for:
- Providing accurate, complete, and honest information about your medical history, symptoms, medications, allergies, and current health status
- Being physically located in a state where Innocre is licensed (Delaware, Maryland, or Washington) at the time of your visit
- Participating in your care by following treatment plans, taking medications as prescribed, and attending follow-up appointments
- Notifying your provider promptly if your condition changes, worsens, or does not improve
- Keeping your scheduled appointments or cancelling/rescheduling in accordance with our cancellation policy
- Providing a safe, private, and quiet environment for your telehealth visit
- Treating all healthcare staff with courtesy and respect
- Asking questions when you do not understand information or instructions
- Maintaining the confidentiality of your own login credentials and account information
- Paying for services in a timely manner as outlined in our fee schedule
- Contacting 911 or going to the nearest emergency room immediately in the event of a medical emergency

4. Language Access
We strive to provide care in a language you can understand. If you require interpretation services, please let us know when scheduling your appointment and we will make reasonable accommodations.
5. Filing a Complaint
