InnoCre offers board-certified telehealth for adults and adolescents aged 12 and up across Pennsylvania, Maryland, Washington, and Delaware. Every visit is conducted by Atul S. Vellappally, DNP, CRNP, FNP-BC — a Doctor of Nursing Practice with full practice authority in all three states. There is no triage nurse, no resident, no rotating provider. You get the same clinician every time.

Visits run a default 30 minutes — long enough to take a real history, review prior labs and medications, and discuss treatment options rather than rushing through a 7-minute slot. The flat self-pay fee is $68 per visit, paid up front so there is no surprise insurance balance later. HSA and FSA cards are accepted; superbills are provided on request for patients who want to seek out-of-network reimbursement from their own insurance.

Prescriptions are sent electronically to the pharmacy of your choice, typically within minutes of the visit ending. Lab orders go to any Quest or LabCorp draw site. Results are reviewed by your provider and communicated through the secure HIPAA-compliant patient portal, where you can also message between visits with clinical questions about your care plan.

What we treat: the 50+ conditions listed below — common acute illness (UTI, sinusitis, strep, pink eye, cellulitis, GI complaints), respiratory and ENT (cold, flu, COVID, asthma, bronchitis), skin conditions, women's and men's reproductive health, STI testing and treatment, and ongoing management of stable chronic conditions like hypertension, diabetes, GERD, hypothyroidism, and high cholesterol.

What we do not treat via telehealth: we do not prescribe Schedule II–V controlled substances, opioids, benzodiazepines, stimulants, antipsychotics, or mood stabilizers. We do not perform pelvic exams, pap smears, or any procedure requiring physical examination. Emergency symptoms (chest pain, difficulty breathing, severe abdominal pain, signs of stroke) are not appropriate for telehealth and should go to the nearest emergency department or 911. Honest triage is part of every visit — if your condition needs in-person care, we will tell you.

How do I know if my condition is appropriate for telehealth?

Most acute conditions where a clinician can take a thorough history, review your symptoms (and a photo when relevant), and check your medication list are appropriate for telehealth. That covers the majority of what brings people to urgent care: UTI, sinus pain, sore throat, ear infection, rashes, GI complaints, allergy flares, and STI screening. It also covers ongoing management of stable chronic conditions like hypertension, diabetes, hypothyroidism, and high cholesterol.

Conditions that are not appropriate include anything requiring a physical exam (pelvic exam, pap smear, joint injection, abscess drainage), suspected emergencies (chest pain, difficulty breathing, severe abdominal pain, signs of stroke), conditions requiring controlled substances, and anything you suspect might need imaging or in-person testing. If you book a visit and your symptoms turn out to need in-person care, your provider will refer you to the appropriate setting and there is no extra charge for the triage time.

What does a visit cost and do you take insurance?

Every visit is a flat $68 self-pay fee, paid at the time of booking. There is no copay, no deductible to meet, no surprise balance billing later. We do not bill insurance directly — this keeps overhead low and pricing transparent.

If you have insurance, you can still use InnoCre. We accept HSA and FSA cards, and we provide an itemized superbill on request that you can submit to your insurer for out-of-network telehealth reimbursement. The amount you get back depends on your specific plan's out-of-network coverage. Many patients find that the $68 flat fee is less than what they would owe after an in-network urgent care copay anyway.

Which states do you serve, and what about traveling between them?

InnoCre is licensed in Pennsylvania, Maryland, Washington, and Delaware. To be seen, you must be physically located in one of those three states at the time of the visit — this is a state licensing requirement that applies to all telehealth, not just InnoCre. Your residency or insurance does not matter; what matters is which state you are sitting in when the video call starts.

This means if you live in Virginia and are visiting Maryland for the week, you can book while you are in Maryland. If you live in Maryland and travel to Florida, we cannot legally see you while you are in Florida. The state at the time of the visit is what counts.

What prescriptions can you NOT send via telehealth?

We do not prescribe any of the following via telehealth, in keeping with federal DEA rules and our clinical scope of practice:

  • Schedule II–V controlled substances — opioids (Vicodin, Percocet, OxyContin, etc.), benzodiazepines (Xanax, Ativan, Valium), stimulants for ADHD (Adderall, Vyvanse, Ritalin), gabapentin in some states, testosterone, ketamine.
  • Buprenorphine for opioid use disorder (Suboxone, Subutex) — requires a specialized provider with an X-waiver and is best managed by a dedicated MAT program.
  • Antipsychotics and mood stabilizers (Seroquel, Risperdal, lithium, Depakote, etc.) — these require ongoing psychiatric monitoring outside the scope of a primary-care telehealth practice.

We can prescribe most other medications, including antibiotics, antivirals, antifungals, antihypertensives, statins, oral contraceptives, non-controlled SSRIs/SNRIs for stable patients on existing regimens, and atomoxetine (non-stimulant ADHD).

How fast can I get an appointment?

Same-day appointments are available most days. Open slots are visible in real time on the booking page — you can see what is available today, tomorrow, and the rest of the week before you commit. If nothing is available within the window you need, the booking system will show the next open slot rather than leaving you guessing.

For truly urgent acute conditions (UTI, suspected strep, sinus infection, cellulitis), most patients are seen within a few hours of booking. For chronic medication refills and routine follow-ups, scheduling a day or two out is usually fine.

Can I get a doctor's note for work or school after a visit?

Yes. If you are evaluated for a condition that warrants time off work or school, your provider can issue a return-to-work or return-to-school note through the patient portal at no additional cost. The note documents the date of evaluation, the diagnosis, and the recommended duration of rest or restricted activity.

We do not issue "doctor's notes" without an actual clinical visit — this is both an ethical and regulatory boundary. If you need a note, book a $68 visit and your provider will write one based on what the evaluation shows.
