A provider-patient relationship is established only after a clinical evaluation has been completed by a licensed Innocre provider during a scheduled telehealth visit.

Important Notice
Innocre Telehealth is not an emergency medical service. If you are experiencing a medical emergency, including but not limited to chest pain, difficulty breathing, severe bleeding, stroke symptoms, or thoughts of self-harm, call 911 or go to your nearest emergency room immediately. Do not use our platform to seek emergency care.
You may be required to create an account through our patient portal, powered by CharmHealth/CharmTracker, to access our services. You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. Please notify us immediately if you suspect unauthorized use of your account.
